This support services department of the school district  provides the required health care for all school age children, grades k-12. We believe a healthy child is better able to receive an education and prepare for a successful future. Listed are some of the services we provide and/or coordinate.
 
REMINDER - Please return a physical form or a dental exam form to your school nurse after your child has their appointments with their doctor or dentist. (Printable version of forms are available under HEALTH FORMS at left menu, from your school nurse, or your school office.)

    State Mandated Dental Screening - Upon entry, Grade 3 and Grade 7
          -Oral Health Education 
          -Dental Referral

Please return a Private Dentist Report to your school nurse after each dental visit. Forms are available at your school or are printable on this site.
