The Harrisburg School District, in partnership with the Highmark Foundation, Highmark Corporation, and Hamilton Health Center, is pleased to announce the opening of a new School-Based Health Center at Downey School.
The goal of the School-Based Health Center is to promote the overall good health of students so they can benefit fully from their educational program.
The permission slip describes the available medical and dental services.
All students attending any District School are eligible to enroll in the
School-Based Health Center.
No child with a permission slip on file will be refused service. Fees for selected services will be billed to the insurers of those students who are covered by health insurance. Medical services can be provided for all current Hamilton Health center families and any child who does not have a regular doctor. If your child has a regular health care provider, limited services are available for illness visits if that provider cannot see your child in a timely manner.
To enroll your child in the School-Based Health Center: complete and return the attached permission slip as soon as possible to your child’s School Nurse. If your child is already enrolled, please complete the form so our files can be updated.
Note: A signed parental permission slip must be on file for your child to receive health & dental services.
